Mini Grants are offered once per year. Apply for up to $500 to support health and well-being in your department!

All Wellness Ambassadors who want to apply for a mini-grant must first:
- Complete the Healthy Department Certification. If you have been granted "striving for certified healthy," "certified healthy," or "gold certified healthy" in the last three years, you will not need to complete this certification process again for FY 2024.
- Conduct a Needs Assessment within the last year. A sample assessment can be found here. You can email this fillable version to your colleagues or you can use some or all of this sample assessment to create your own via Survey Monkey, Qualtrics, or another tool that works for your department.
- Speak with department Account Manager/Business Office to confirm that the purchasing process outlined through this link is feasible. If awarded a grant, this person will be included on all mini grant communication.

FY 2024-25 Grant Awardees:
Once mini grant activities have been completed, please submit an Outcome Report detailing how your grant activities went. The report can be completed via this Form on Smartsheet.
If you have photos from your Mini-Grant activities, you can include them on your Smartsheet form submission or email them to wellbeing@ucdavis.edu.
