Report work-related injuries or illnesses as soon as possible. For life-threatening situations, call 9-1-1 immediately.
Important: Report all injuries to Worker's Compensation within 24 hours. Report serious injuries immediately.
Employees: notify your supervisor right away if you are injured or become ill at work. Your supervisor can help you complete an Employee Injury/Illness Report (EIIR). If you need medical treatment, your supervisor or department safety coordinator will help you get treated at Occupational Health Services or with another treatment provider.
Supervisors: once an employee has reported a work-related injury or illness, assist them in completing the Employee Injury/Illness Report. It’s best for the employee to complete the EIIR themselves, but if that isn’t possible or practical, the report should be based on the employee’s account of what happened. If the employee needs medical treatment, assist them in contacting Occupational Health Services. If an employee is injured outside of regular work hours, you can direct them to another medical provider.
Employee Injury or Illness Report
The Employee Injury/Illness Report (EIIR) application allows any UC Davis affiliate with a valid single sign-on to report a work-related injury or illness. Access the application at app.riskandsafety.com/incident/. Upon reaching the EIIR website, you will be prompted to login using your UC Davis email, CAS credentials and DUO authentication. When you have logged in successfully, you’ll see the following screen prompting you to complete a report:

Completing and saving the Employee Injury/Illness Report notifies Workers’ Compensation and the relevant department contacts of the illness or injury. Workers’ Compensation will then contact the employee to contact Occupational Health (if they haven’t done so already) and give them the opportunity to file a claim
IMPORTANT: Injuries should be reported to Workers’ Compensation within 24 hours. Report serious injuries immediately.
Reporting by Phone
Sometimes the Employee Injury/Illness Report may not be easily accessible. In that case, you can contact the University’s insurance carrier Sedgwick CMS directly by phone at 877-682-7778.
Before reporting, make sure you have notified your supervisor of your injury or illness.
Reporting Serious Injuries or Illnesses
An injury or illness is considered serious if:
- Inpatient hospitalization is required
- An employee suffers an amputation
- An employee suffers the loss of an eye
- An employee suffers any serious degree of permanent disfigurement
While most injuries and/or illnesses can be reported within 24 hours, there is a much higher standard for serious injuries or illnesses. Cal/OSHA requires us to report these immediately, but no later than 8 hours after we find out about the injury or illness. Failure to report on time could lead to at minimum $5,000 in fines for the department.
If there’s a possibility that an employee’s injury or illness is serious, call EH&S immediately at 530-752-1493 business hours. After hours, call Police Dispatch at 530-752-1230. Be ready to provide the following information:
- Name and address of injured employee
- Description of accident
- Nature of injury
- Where employee was taken for treatment
- Accident date and time
- Accident location (the physical address)
- Contact person name and information
Since most serious injuries involve an employee going to a hospital, be sure to contact an emergency contact if one is listed and keep in touch with them to stay updated on their status. Ideally, someone from the department should accompany an injured employee to the hospital.
Once you have contacted EH&S or Police Dispatch to report the accident, complete the Employee Injury/Illness report as soon as possible.
