DEC. 12 Email Notifications
Many Davis campus employees received emails from our new electronic health record system, Enterprise Health. These emails, sent from noreply@enterprisehealth.com, were directed to employees who previously received services from Occupational Health or verified their vaccination status in Enterprise Health. The link in the email is safe and leads to your Enterprise Health portal, where you can review past immunizations and other records. Some employees did receive actionable emails prompting them to complete assigned medical questionnaires in ucdavis.enterprise.health. Vaccination Deadline Nov. 1

UC Davis employees and students have until Nov. 1 annually to comply with UC policy by either uploading their flu and COVID-19 vaccine information or submitting the declination form.

  • Steps for Reporting COVID-19 Vaccination
  • 1.  Select the tile titled “COVID Vaccination” to open a questionnaire.

    2.  Review the definition of Up to Date in the link. If you are up to date, select “already received the vaccine.” You will see a link titled “ADD AN IMMUNIZATION.” Begin typing the vaccine name in the legend to guide you in selecting the updated vaccine you received. Please don’t enter vaccines that do not meet the definition of Up to Date in the link.

    3.  That will complete your questionnaire and a timestamped copy of your submitted questionnaire will appear under “Messages” for your records.
  • Steps for Declining COVID-19 Vaccination
  • 1.  If you have not received an updated vaccine and do not plan to receive one, select “I decline Receiving the vaccine.” 

    2.  Please review the declination and links, and select the green submit box.

    3.  That will complete your questionnaire and a timestamped copy of your submitted questionnaire will appear under “Messages” for your records.
  • Steps for Reporting Flu Vaccination
  • 1.  Select “Flu Vaccination” from the portal welcome screen to open a questionnaire.

    2.  If you already received your vaccine, select “Already Received Vaccine.” You will see a link titled “ADD AN IMMUNIZATION.” Begin typing the vaccine name from the legend to guide you in selecting the vaccine you received. If you received an injectable vaccine, select Flu, Preservative Free, which is the most common injectable flu vaccine. You can also select intranasal if you received a nasal flu mist. Confirm that you added your immunization, and select the green submit box below.

    3.  A timestamped copy of your submitted questionnaire will appear under “Messages” for your records.
  • Steps for Declining Flu Vaccination
  • 1.  If you are declining the influenza vaccine this season, select “I decline receiving the vaccine.” 

    2.  Please review the declination, read and initial in each box, and select the green submit box below.

    3.  A timestamped copy of your submitted questionnaire will appear under “Messages” for your records.

Top Tips for Enterprise Health
  • I recently had COVID-19. What should I do? The CDC recommends that those recently infected with COVID-19 should refrain from a COVID-19 vaccine for 90 days. If you recently had a COVID-19 infection and will not get the updated vaccine before Nov. 1, you should submit the declination form through the portal.

COMING SOON: Features of Enterprise Health

More functionality is being enabled in Enterprise Health to streamline managing employee health records and appointments, transitioning the Davis campus from an email-based system to a secure, comprehensive portal.

For Employees:
Employee's will receive email notifications for
  • Appointment reminders and when health services are due
  • Questionnaires that need to be completed on the portal
 
  • NOTE:
    • Work Related Illness/Injury: use the Employee Injury and Illness Reporting System.
    • Contact w/ Live Vertebrates: use the Occupational Health Surveillance System (OHSS). 
For Supervisors:

Supervisors will have access to a portal to request services (e.g., work clearances, respirators) on behalf of the employee instead of by email, fax or calling.

  • Supervisors use their portals to:
    • Pull reports to see if employees are due for or have completed required services
    • Complete Request and Authorization forms for required services on behalf of your employees
    • Automatically Receive copies of employee Work Clearances and Work Restrictions
