  • Any spill of recombinant DNA or infectious agents must be reported as soon as possible to the Biological Safety Officer.  Consult EH&S SafetyNet #127 "Biological and Biohazardous Spill Response" for further information.
  • Report all injuries and accidental autoinoculation, ingestion, or inhalation of infectious agents to the laboratory director or supervisor, EH&S (use the form below or call 530-752-1493) and the UC Davis Occupational Health physician (530-752-6051) for evaluation and possible treatment.
  • Any accidental release of recombinant plants or plant pathogens to the environment must be reported to the Biological Safety Officer (use form below or call the Biosafety Office).

Incidents should be reported to the principal investigators (and laboratory supervisor), as well as to the UC Davis Biological Safety Officer.

Other entities or agencies may also need to be notified (e.g. IRB, IACUC, NIH RAC, USDA-APHIS, CDFA), if required.
