There is NO CHANGE to reporting requirements
Work-related injuries or illnesses must be immediately reported

Why is this change happening?

The system behind our current Employer's First Report is outdated and challenging to maintain. The new Employee Injury and Illness Reporting tool uses a newer platform that simplifies navigation, improves maintenance by administrators and could possibly allow for some customizations in the future (e.g., expanding use beyond employees).

The new Employee Injury and Illness Reporting tool asks for all the same required information as our current tool, but it navigates the user through reporting in a streamlined and simplified manner.

What will this change entail?

  • On Monday, May 29 all references to Employer's First Report on the Safety Services website will be changed to Employee Injury and Illness Reporting
    • The URL to the Employee Injury and Illness Reporting will be posted online and the legacy URL to the Employer's First Report will be redirected
  • On Monday, May 31 all UC Davis employees will gain access to the Employee Injury and Illness Reporting tool
