Animal Care & Use Occupational Health Program

This program is an integral part of Occupational Health Services and is designed to provide preventative medicine services and health-related education to those employees who have contact with live vertebrate animals in the course of their employment at UC Davis. Notice to students enrolled in "199" classes.

 

Program Elements

  • Risk Assessment of the work site
  • Medical assessment of individual health history (Health Surveillance Questionnaire)
  • Diagnostic tests and immunization (if required)

 

Online Form Submission

As of December 1, 2007, the Risk Assessment (RA) and Health Surveillance Questionnaire (HSQ) forms are submitted through an online application, and paper forms are no longer accepted. For help with the online forms, read the instructions, then call Occupational Health at 530-757-3200 if necessary.

Participants must wait for the automatically generated email notification before they can complete the HSQ.

 


SURVEY 

Please take this short survey to provide comments, suggestions, or complaints about the new online application process.

 

Follow-up

Once the forms are submitted, participants will receive an evaluation and recommendation letter via email from the reviewing physician at Occupational Health. Possible outcomes include:

  • No further services required for 3 months, 6 months, or one year.
  • Appointment for consultation and further assessment is recommended. The Participant must obtain authorization and a recharge account prior to making an appointment at Occupational Health Services.
  • Immunizations, respirator fit test, or other services may be recommended based on risk and health history.
