How to Report a Concern

Procedures for Completing the "UCD ANIMAL CARE AND USE REPORTING FORM"


The UCD Institutional Animal Care and Use Committee (IACUC) has approved the following instructions and procedures for reporting animal care and use concerns.

  1. All reports must be submitted on the “UCD Animal Care and Use Reporting Form” (version 03/2007 - MS Word format).

  2. If you have identified an animal care and/or use concern(s), please complete Part A of the “UCD Animal Care and Use Reporting Form.” If you prefer to describe your concerns in person or by phone, you may contact the IACUC office and an IACUC staff member will complete the report (Part A, Reporting Form) before investigating or passing this on to the person investigating the concern. If you are reporting a concern and wish to remain anonymous, then do not enter your name in section A.3. , “Person Reporting Concern.”

  3. Once Part A is completed, the form should be forwarded to the IACUC staff office for review and action. If immediate or urgent veterinary care is required, the report will also be forwarded to the Attending Veterinarian.
