FUNDING REQUEST INSTRUCTIONS

 

Be Smart About Safety (BSAS) is a safety and loss prevention program initiated through the University of California Office of the President, Risk Services (OPRS) and will begin on July 1, 2009 for fiscal year 2009-10. The program is designed to provide a funding mechanism and increase safety awareness  by enabling investment in loss prevention programs, equipment and training/promotional efforts in order to reduce the probability of employee injury/illness. There is no deadline in applying for 09-10’ BSAS funds but requests will be considered on a ‘first come, first served’ basis and if there is a conflict for remaining funds, based on the risk.

INSTRUCTIONS FOR COMPLETION OF FORM(s)

There are only 2 BSAS forms (Call Bob Wachter 530-297-4605; rawachter@ucdavis.edu for 'any' assistance needed) 

  • The Be Smart About Safety Funding Request form (upper left corner link) which must accompany ALL applications for BSAS fund requests (inc. office ergonomic equipment), and
  • The Office Ergonomic Equipment request form (upper left corner link) for office, workstation equipment

 

Approved non-office equipment is funded at 100% and office ergonomic equipment at 50% up to $500/employee as a ‘shared cost’ with the department.

 

BE SMART ABOUT SAFETY “FUNDING REQUEST FORM”:

  1. Be Smart About Safety (BSAS)  funding requests can be submitted by any department with injury prevention needs due to hazard/risk exposures, including Emergency Planning, Police and Fire Chiefs, Housing, Dining and Facilities. All departments are encouraged to apply for funds for injury prevention and risk mitigation.  The fund request form is available at the Occupational Health website on the Be Smart About Safety page: http://safetyservices.ucdavis.edu/occupational-health-services/bsas/FundRequest/view

 

  1.  BSAS funds are typically requested for the following types of injury reduction purposes;

·         Correct safety deficiencies, reduce risks

·         Provide employee safety training & development

·         Purchase safety equipment and supplies

·         Fund consultants for safety and injury prevention needs

·         Initiate new safety or wellness programs

·         Address human and property security concerns

·         Correct deferred maintenance issues that directly impact life safety

·         Fund capital improvements directly related to life safety (change from 07-08)

  1. BSAS are not available or approved for;

·         Enhancements to academic or vocational instruction

·         Supplement program budgets; 

·         General operating supplies expenses

·         Personal protective and safety equipment required by law or regulation

·         Travel expenses (with limited exception for train-the-trainer safety education)

·         Replacement of stock items for ongoing programs

·         Pay salaries/wages for existing positions

 

 

  1. Complete the “Be Smart About Safety 2008-09 funding request form” (Excel sheet; upper left corner link) in its entirety.  Incomplete or illegible forms may not be processed.  Provide all required signatures before submission. Attach any supporting documentation (limit to 1 page); i.e. further details supporting the injury/illness prevention impact, pictures of equipment being request, web-links, the purchase quote, catalog page, etc. NOTE: The minimum request amount is $200.

 

  1. NOTE: You must submit a Dafis number shown under Ops Fund #69790 or your funding request form or will not be reviewed

 

  1. You must answer all questions on the form and have all 3 printed names & signatures required at the bottom of the form, the department head’s signature at the bottom of the form is an agreement to meet all 4 BSAS funding terms

 

 

OFFICE ERGONOMIC EQUIPMENT REQUEST FORM:

  1. If you are completing an “Ergonomic (office work station) requests forms” http://safetyservices.ucdavis.edu/occupational-health-services/bsas/ErgoEquipRequest/view for individual employee work station equipment you must complete one form per employee and the  total funding requested must equal the total requested on the  “Be Smart About Safety Funding Request Form). Be sure you include tax and shipping costs.

 

  1. Office work station funding request must have three items; 1) The ergonomic request form for the employee listing what equipment is requested and the cost(s), 2) a Tier I evaluation form supporting the equipment request http://safetyservices.ucdavis.edu/occupational-health-services/bsas/ErgoTierOne.pdf/view , and 3) an ergonomics quiz completed by the employee for this the work station http://safetyservices.ucdavis.edu/occupational-health-services/bsas/ErgoQuiz/view. NOTE: The minimum request amount is $100/employee work station request.

 

  1.  Requests for ergonomic related office equipment purchases need to be accompanied by the “Ergonomic Equipment Request” form. The cost of these purchases will be a shared responsibility with the department. Fifty (50%) of the total cost must be supported by the requesting department, and each person will be allotted no more than $500 from the BSAS funds. 

 

FUNDING REQUEST SUBMITTAL:

  1. Return all forms via regular inter-campus mail or hand carry to 501 Oak Avenue, attention Bob Wachter (530-297-4605). Electronic submissions must include required signatures and can also be sent to rawachter@ucdavis.edu.

 

  1. Call Bob Wachter, 297-4605 or Marcy Best, 757-3262 with questions regarding completion of these forms.

 

  1. You will be notified by e-mail of the approval or disapproval of your request and will have 4 months to use approved funds.  Fund transfers to the departmental Dafis account will be administered by OOA, Safety Services; (pjhodge@ucdavis.edu).

 

  1. You will have 4 months from the date of authorization to use these BSAS funds
