Hand-held Torch Replacement
In September 2008, two UC Davis employees were severely burned while working with a hand-held gas compressed gas cylinder and torch nozzle. Based on investigation of the accident and the failed cylinder/nozzle assembly, it was determined that the design of the hand-held gas cylinder assembled with the torch nozzle is susceptible to bending stresses at the neck of the cylinder. Bending stresses can result from impact during use or during transportation with the torch nozzle attached to the cylinder. As was the case in this accident, once bending stresses at the neck of the cylinder occur, there is potential for the cylinder/torch assembly failure and resulting ignition of flammable gas.
In order to prevent future accidents related to use of this equipment, EH&S recommends the following actions be instituted UC Davis-wide:
- Discontinue the use of all hand-held gas cylinder and torch nozzle assembly.
This prohibition has been reflected in section 290-86 Hazardous Operations, of the Policy and Procedure Manual.
- Replace all hand-held gas cylinder and torch nozzle assembly with newly designed units that incorporate a separate cylinder and hose/torch assembly. This design reduces the possibility of bending stresses weakening the neck of the cylinder.
- Properly train all employees on the use of the new units.
Be Smart About Safety program funds have been earmarked to expedite this replacement process. Safety Services has purchased newly designed BernzOmatic Trigger Start Hose Torch and Fat Boy cylinder units. This unit includes a high intensity swirl flame, instant on/off trigger igniter, and adjustable flame control, with a five-foot hose, wide-body cylinder, and cylinder holster for maximum mobility and accessibility. These new units, normally priced at over $125 are available for a $30 campus recharge with the exchange of a hand-held cylinder and torch assembly. Units are also available without exchange for $60. This exchange program has been extended and will be in effect until December 31, 2009.
